Required Day Zero Registration Forms

Riders and Crew Members will turn in the following forms at Day Zero Registration on Wednesday, August 1. This will include reading the Medical Crew Letter. To save time, you are encouraged to read this and fill out the forms ahead of time. If you have any questions before Day Zero, please contact the Ride Coordinator. The Medical Crew Captain will be your contact person for turning in all medical forms at Day Zero registration.

The ACT 10 Medical Crew will be staffed by two physicians (Medical Directors), the Medical Crew Captain (an RN), and a number of other health care professionals. The Chiro/Massage Crews will be staffed by licensed chiropractors, massage therapists, and massage students.

Medical History Form

The ACT 10 Medical Crew will need to know the past and present medical conditions of all Riders and Crew, in case of an emergency requiring treatment.

Medication Policy Form

If you will need cold storage for any medication on the Ride, please fill out the Medication Form.

Medications at Day Zero

Remember to bring any medications you will have on the Ride to Day Zero. This includes prescription and over-the-counter medicines. Medical Crew will provide cold storage if needed.
